Job Description

Title:   Social Work Care Manager Outpatient                                                                                                 

Department:  Integrated Care Management                        

Reports To:    Manager, Case Management Social Work

                                                

Position Summary:  As part of the outpatient multidisciplinary team, provides assessment and intervention to assist clients/families to improve social and economic difficulties interfering with health and wellness through use of casework process and principles, strategies, and community resources.  Receives referrals for individuals from at-risk populations from interdisciplinary team members within the outpatient clinic setting. 

Essential Functions and Responsibilities as Assigned:

Collaborates with the outpatient multidisciplinary teams to determines nature of client's situation by interviewing client; assessing medical, psychological, emotional, and social information; making on-site visits.

Establishes course of action by exploring options, setting goals with client.

Obtains assistance for client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.

Fosters client's action or adjustment by interpreting attitudes and patterns of behavior, explaining and pointing out new options as patients are presented with new diagnoses and treatments options within the outpatient clinic setting.

Performs individual and/or family therapy, as indicated.

Maintains Medical Record and department case documentation barriers to goal attainment, and client/family response to intervention.  Maintains appropriate casework statistics.

Monitors planned actions by periodic follow-up.

Maintains effective operations by following policies and procedures; participating in quality reviews; reporting needed changes.

Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs.

Participates in interdisciplinary conferences and provides consultation for clinical staff.

Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.

Represents the integrated care management department on various teams and performance outcomes committees and projects.

Performs other related duties as required and directed.

Qualifications:

Required:

Master’s degree in social work from accredited educational institution

State licensure or eligibility for licensure as a Master Social Worker (LMSW) and maintenance of continuing education requirements

Preferred: 

Basic Life Support (BLS) certification as a Healthcare Provider by the American Heart Association, American Red Cross or equivalent through the Military Training network (MTN)

Oncology or medical social work experience

Internship completed at a medical facility

Certification in Case Management Certification (ACM or CCM)

Three years acute hospital care or social work experience

Knowledge, Skills, and Abilities:

Ability to handle crisis situations and to act calmly and effectively in busy/stressful environment.

Ability to handle multiple tasks, organize, prioritize and complete work within short timeframes.

Ability to manage patients with different types of personalities.

Ability to spend majority of time utilizing a computer, monitor, keyboard and mouse; ability to type.

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. 

Ability to work with diverse teams and individuals, team player.

Competency in applying the principles, methods, materials, and equipment necessary in 
providing case management services.

Compliance with the Code of Ethics and Guide for Professional Conduct.

Current knowledge of treatment theories, practices, and techniques per discipline.

Customer-focused attitude and ability to work proactively and efficiently with appropriate urgency.

Demonstrated ability to read and interpret documents such as safety rules, operating and maintenance instructions, and policy and procedures manuals. 

Demonstrated leadership skills through professionalism as a team player, excellent observation, communication, and decision-making abilities.

Knowledge and ability in utilizing computerized hospital/health information management systems and software applications (i.e., Cerner, Optum etc.) and Microsoft Office Suite applications.

High degree of interpersonal skills to effectively communicate with physicians, nursing, families/patients, utilization management and internal and external customers. 

Maintains knowledge of applicable CMS rules and billing regulations related to Medicare, Medicaid, and commercial insurance.

Must be able to work independently, and utilize problem solving techniques and critical thinking skills.

Weekend, holiday, and evening coverage as per site requirements.
